Error code identifier. Budget misses (integration or fan-out did not answer within settings.timeout) are ERR_CODE_TIMEOUT. Transient provider-side failures (HTTP 502/503/504, connection error or open circuit breaker) are reported as ERR_CODE_BAD_RESPONSE.

Budget misses (ERR_CODE_TIMEOUT) also set announced_timeout_ms to the timeout that was applied (settings.timeout, in milliseconds).
